A form label needs to survive the first keystroke

Keep each field's purpose clear while someone is entering their information.

Name what the field asks for

Read each form label as a question. Would a visitor know what to enter without remembering the page's introductory paragraph? 'Details' may be too broad when you need a short description of the requested service. Choose labels that match the actual task and distinguish similar fields. W3C guidance calls for form controls to have associated labels. A visible arrangement that looks understandable is not by itself evidence that the label is correctly connected for assistive technology.

Separate examples from the field name

An example can help a person understand the expected input, but it should not be the only way they learn the field's purpose. Write a stable label and add concise supporting instructions where needed. Avoid asking for several unrelated items in one unexplained box. Before collecting additional information, identify the decision or action it supports and whether it is appropriate to request at this stage. More information is not automatically more useful if the business cannot explain why it needs it.

Test the completed interaction

Enter harmless fictional data in a preview and check whether every field remains understandable after typing begins. Ask a developer or accessibility specialist to verify the label associations and relevant keyboard and assistive-technology behavior. Record a concrete problem rather than declaring the whole form accessible after a quick visual review. Finally, check the submission and acknowledgement path. A clear field label helps with one part of the task; people also need to know whether their request was received and what happens next.
